SS-31 (also known as MTP-131 or elamipretide) concentrates 1000-fold in the inner mitochondrial membrane by binding to cardiolipin, a phospholipid unique to this membrane. This binding stabilises cardiolipin-cytochrome c interactions, improving the organisation of cristae junctions and electron transport chain supercomplexes. The result is improved mitochondrial ATP output, reduced electron leak and ROS production, and protection against mitochondrial permeability transition pore opening during ischaemia.
