What is the difference between HMG and recombinant FSH?

HMG is extracted from urine of postmenopausal women and contains both FSH and LH. Recombinant FSH (rFSH: Gonal-F, Puregon) is produced in CHO cells and contains pure FSH only. In most IVF protocols, rFSH is preferred for ovarian stimulation because it provides precise FSH dosing without variable LH. However, HMG is still used when both FSH and LH activity are desired, and some research indicates HMG improves live birth rates in certain populations.

When is HMG used instead of hCG for male fertility research?

hCG provides LH-like activity only. HMG provides both FSH and LH activity. For restoring complete spermatogenesis (not just testosterone production), FSH is required to support Sertoli cells - particularly for sperm maturation. Research protocols for hypogonadotropic hypogonadism often start with hCG to restore testosterone, then add HMG (or rFSH) to complete spermatogenesis restoration.
